The medical anthropologist, Leslie Farkas, determined the average dimensions and relations of facial landmarks and features of thousands of people from various ethnic and other study groups. In this study of the healthy North American Caucasians, he found that, on the average, most people have a convex face. Relatively few had a midface which is concave.
Many people think that a concave midface is less attractive than one which is convex. The lack of skeletal projection provides less support for the lower lids and cheeks. The lack of skull projection in patients with concave midface provides less support for the lower lids and cheeks. This leads to their premature descent with premature aging as evidenced by early bags, scleral show, and lack of cheek fullness. In addition, concave midface makes the eyes and nose appear more prominent.
Central midface concavity is not effectively treated with fillers, fat grafts, or standard cheek implants. To make a concave midface appear convex requires skeletal augmentation of the midface with multiple implants, as well as midface soft tissue resuspension (subperiosteal midface lift). This patient underwent central midface augmentation with multiple custom-caved implants. They included paranasal, infraorbital rim, and medial malar implant. A subperiosteal midface resuspension was also performed.

This patient desired several changes in his facial contour. In two operations, Dr. Yaremchuk performed infraorbital rim (lower eye socket), paranasal (bones lateral to the nose), malar (cheekbone), and mandibular body (lower jaw) augmentation.
